Office PolicyThe following is an explanation of our office policies. We believe that a clear definition will allow us both to concentrate on the most important issue: Regaining and Maintaining your Helth. We will be happy to answer any questions you may have regarding our policies, your account or insurance coverage. All questions related to your health will be directed at Dr. Jim. Bear River Chiropractic will provide a special "courtesy" consultation, or brief conference with anyone interested in finding out if chiropractic care can help them with their individual health problem or concern. There is NO charge or obligation in connection with this appointment. Financial PolicyWe feel the patient's health needs are paramount; therefore the following payment policy is an attempt to allow you, the patient, to receive the care you need and clear your balance with the least amount of difficulty. 100% of the first visit charges are due and payable at the time of service. The majority of our patients do not have chiropractic health care benefits. Our goal is to contain your health care costs and to provide you with the care you need or the care you think you can afford. Therefore, the Doctor will always discuss any special fees for services. Patients under care are required to make regular payments on all unpaid balances. Payments need to be made at the time of service. We do NOT send monthly statements, but we are happy to provide you with an itemized statement for your records/taxes. Today many health insurance policies have provisions for chiropractic care. We will be happy to partner with you to electronically file your Primary insurance claim and do everything we can to assure you receive proper reimbursement. However, we CANNOT take responsibility for what your health insurance will or will not cover. You will be responsible for covering the estimated patient portion at the time of service. If there is any outstanding balance it will be your responsibility. Thank you in advance for understanding. If you have a legitamate financial hardship, please discuss your concerns with us. Perhaps you are eligible for a special payment program designed for these unique situations. MEDICARE will cover 12-18 visits per year once you have satisfied your yearly deductible. Medicare will only cover a portion of the usual and customary fees. Medicare does NOT cover examination procedures, x-rays, supplements or supports, so we ask that you take care of these at the time services are rendered. Appointment PolicyIn order to better serve our patients we ask that you call 24 hours in advance if you are unable to make your scheduled appointment. Your appointment time is reserved for you. If you fail to notify our office, it leaves a time slot open that could be used to help someone else. More importantly keeping your appointments allows you to get desired results and avoid a "$25 missed appointment fee". |
